Alumni Questionnaires
Below you will find the links to the Alumni Qestionnaires. Please click on the questionnaire that coincides with the degree that you received from EKU Department of Biological Sciences. These forms have been created in dynamic pdf format which means that they can be filled out on-line. You will need to have Adobe Acrobat Reader, version 6.0 or later in order to fill out these forms. If you do not have this version, you can download it from Adobe.com and follow the installation instructions.
Once the form is completed, click the "Submit by email" button at the bottom of the questionnaire. Follow the online instructions for submitting the document. These questionnaires should be mailed to: Charles.Elliott@eku.edu.
